REQUIREMENTS

Insurance Documents

  • Workers Compensation Certificate – Statutory (or exemption certificate)
  • Employer’s Liability – $1,000,000
  • Comprehensive General Liability – Such insurance shall cover liability arising from premises, operations, contractual liability and products and completed operations. Minimum limits as follows: $1,000,000 Per Occurrence, $2,000,000 General Aggregate, $2,000,000 Products and Completed Operations. Must list all vehicles on the certificate and list Riverside Contracting, Inc. as Additionally Insured
  • Comprehensive Automobile Liability Insurance – Must cover all owned, non-owned, and hired vehicles used in connection with the work, with the following minimum limits: Bodily Injury/Property Damage – $1,000,000 Combined Single Limit.

Certificates of Insurance will be provided by the Vendor to the Contractor “Riverside Contracting, Inc.” prior to the commencement of work, and thereafter as required by the Contractor to verify coverage. Contractor will be named as an additional insured for Comprehensive General Liability and Comprehensive Automobile Liability and this will be stated on the Certificate of Insurance. The Certificate of Insurance will also contain a Waiver of Subrogation in favor of Contractor.

  • Copy of Vehicle(s) Registration
  • Current Driving Record
  • Current W-9
  • Executed Annual Contract (Riverside provides upon processing)
